| This plan sets you up for Bi-Monthly Payments for the Second Year of Florida Licensure, following the First FREE Year. It includes the first 2 Bi-Monthly Payments. |

| Second year DC - 1 Year - $258 |
| This plan is the Yearly Payment for the Second Year of Florida Licensure, following the First FREE Year. |

| Third year DC - Bi Monthly - $58 |
| This plan sets you up for Bi-Monthly Payments for the Third Year of Florida Licensure. It includes the first 2 Bi-Monthly Payments. |

| Third year DC - 1 Year - $348 |
| This plan is the Yearly Payment for the Third Year of Florida Licensure. |

| FCA Member DC - Bi Monthly - $73 |
| This plan sets you up for Bi-Monthly Payments for a D.C. past the first three years of Florida Licensure. It includes the first 2 Bi-Monthly Payments. |

| FCA Member DC - 1 Year - $438 |
| This plan sets you up for Yearly Payments for a D.C. past the first three years of Florida Licensure. |

| Associate Member - Bi Monthly - $33 |
| This plan sets you up for Bi-Monthly Payments for Associate Member. This is for a licensed chiropractic physician not practicing in the state of Florida; or a lay person in accord with the principles and objectives of the FCA (exhibitor, supplier, advertiser, chiropractic assistant, etc.). It includes the first 2 Bi-Monthly Payments. |

It includes the first 2 Bi-Monthly Payments. |
